I can at least answer that because I've known people who played dota for 6 years and were still autoattacking. They just were never told or saw how to do it right ---not by anyone they believed anyway. Sometimes they would get some angry person flaming them...but most people simply never think of anything themselves. Not just in dota, but in everything. If they do not see it done they cannot do it.
These people who I played with in dota 1 did not seem especially stupid. They were just ordinary people who played they game badly together and never knew any other way to do it. After 200 hours they may have never purchased a ward, may not know how to find them in the shop...lots of people just can't figure things like that out for themselves.

So based on the handful of teams I've been on and DBR and such I figure my MMR must be around something like top 2 or 3%. But I was in a game last night where there was a guy who is on a team with 1600 tMMR, he must be near the bottom 2 or 3%. This was in the solo que...I'm sorry to say I accused him of throwing. I could not imagine someone that bad being in my game. He played storm and somehow fed 5 kills to the opposing mid solo invoker in 8 min. But maybe he really was that bad. Maybe the matching can really be that terrible.
If I just knew at the start that he was horrible at dota I could have at least insisted that he let me take mid and had him go hide somewhere so he would not feed much. I could have tried to get him to take an easier hero, maybe an SK or something. But there is no way to know until you see the people play.

So, a Phantom Assassin and myself were left in this mess, and we decided to piss the enemies off and we didn't left the game. What I have to admit though is that all this happened whilst me being muted last night (Wasn't justified, I just pointed some things out to our 0-13 visage).
